We are seeking a dedicated and highly organized Long Term Care Specialist to join our team at Buffalo Center in Buffalo, NY.

About the Role

This is a challenging yet rewarding position that requires strong clinical skills, attention to detail, and excellent communication abilities. As a MDS Coordinator, you will be responsible for completing accurate assessments, care plans, and supporting activities assigned by management.

You will work closely with our interdisciplinary team to ensure the highest quality of life for our residents, maintaining their dignity and independence while promoting a warm and nurturing environment.

Responsibilities
  • Completing MDS assessments and care plans accurately and efficiently
  • Initiating care plans and supporting activities as assigned by management
  • Maintaining and updating all care plans and assessments in a timely manner
  • Monitoring and auditing clinical records to ensure accuracy and timeliness
  • Informing management of persistent issues related to non-compliant documentation
  • Protecting the confidentiality of resident and facility information at all times
Requirements
  • Prior experience with MDS 3.0 required
  • Valid New York RN License required
  • Long-term care experience essential
  • Strong organizational, professional, and motivated individual needed
  • Solid computer skills and excellent communication abilities required
  • Friendly and team-oriented approach preferred
