Job Description:

We are seeking a skilled Patient Care Coordinator Specialist to join our team at Della Infotech.

About the Role:

This is a full-time position that involves working closely with patients, families, and healthcare teams to ensure high-quality patient care. The ideal candidate will have excellent communication skills, be able to work independently, and possess a strong understanding of patient health care plans.

Key Responsibilities:
  • Assess New Patients: Conduct thorough assessments of new patients to identify their health care needs and develop personalized care plans.
  • Update and Revise Patient Health Care Plans: Continuously evaluate and update patient health care plans as needed to ensure they remain effective and relevant.
  • Educate Patients and Families: Provide clear and concise information to patients and their families about their health care plans and options, empowering them to make informed decisions about their care.
  • Make Clinical Decisions for Routine Patient Care: Utilize clinical expertise to make sound decisions regarding routine patient care, ensuring the best possible outcomes.
  • Accurately Document and Submit Medical Records: Maintain accurate and detailed medical records, submitting them in a timely manner as required.
