Job Description:
We are seeking an experienced RN/LPN to join our team as a Home Health Care Nurse in New Castle. As a Home Health Care Nurse, you will provide one-on-one care to patients in their homes, ranging from 8 to 16-hour shifts. You will work closely with our patients, families, and healthcare teams to develop and implement individualized care plans that meet each patient's unique needs.

Responsibilities:
* Implement physicians' prescribed plans of care for patients
* Perform patient assessments and coordinate care with other healthcare professionals
* Administer prescribed medication, treatments, and therapies as needed
* Document all care provided in our electronic medical record system
* Educate patients and family members on health promotion, disease prevention, and management of chronic conditions
* Collaborate with case managers and other healthcare professionals to ensure seamless transitions of care

Requirements:
* Current RN or LPN licensure in the state of employment
* At least six months of experience in home health care or a related field
* Strong communication and interpersonal skills
* Ability to work independently and as part of a team
* Valid driver's license and reliable transportation
