About the Role:

As a Medical Care Coordinator for Vulnerable Populations with Redwood Family Care Network, you will play a critical role in ensuring the health and well-being of individuals with intellectual and developmental disabilities.

We are seeking a highly skilled and compassionate individual to join our team as a Medical Care Coordinator. In this role, you will be responsible for coordinating medical care for clients, including scheduling appointments, communicating with healthcare providers, and managing medication regimens.

You will work closely with our interdisciplinary team to ensure that each client receives high-quality medical care tailored to their unique needs. Your excellent communication and organizational skills will enable you to effectively coordinate care across multiple settings, including clinics, hospitals, and community-based programs.

About You:

To be successful in this role, you should have a strong background in medical coordination, experience working with vulnerable populations, and excellent communication and organizational skills. A Certified Medical Assistant License or Licensed Practical Nurse certification is preferred, but not required.

You should be able to lift and transfer up to 50 pounds and be comfortable working in a fast-paced environment with frequent interruptions. Our ideal candidate is passionate about making a positive impact in the lives of others and is committed to delivering exceptional care to our clients.

What We Offer:

We offer a competitive salary range of $67,724.80 - $73,000 per year, depending on experience, plus an on-call stipend.
