Job Title: Patient Navigator/Driver
Job Summary: We are seeking a highly motivated and detail-oriented Patient Navigator/Driver to join our team at Gateway Community Health Center, Inc. The ideal candidate will have a strong background in social work, sociology, or a related field, and will be able to work independently and as part of a team to provide field-based navigation services to patients enrolled in our Community Mental Health Grant Program.
Key Responsibilities:
- Work closely with the interdisciplinary team to assess and recognize the necessary treatment and management for patients.
- Develop and maintain case files for each patient, identifying primary care and mental services needed, and facilitating referrals as necessary.
- Provide peer support group services, health education, skills development, and medication management as needed.
- Ensure a smooth transition for patients from one healthcare provider or setting to another, minimizing fragmentation in their medical care.
- Evaluate patients using the Patient Health Questionnaire-8 for children and Patient Health Questionnaire-9 for adults.
- Assess patients through the PRAPARE Assessment to identify social determinants of health.
- Maintain confidentiality of patient information per HIPAA guidelines.
Requirements:
- Bachelor's degree in Social Work, Sociology, Psychology, Health Education, or a related field from an accredited college or university.
- College education may be substituted for three years' experience in a healthcare setting.
- Ability to work long hours during the week and weekends as necessary.
- Bilingual in English and Spanish is preferred.
- Possess means of transportation and a valid Texas Driver's License.
