Job Summary

We are seeking a skilled Occupational Therapist to join our Home Health team. As an Occupational Therapist, you will play a vital role in helping patients regain their independence and improve their quality of life.

Responsibilities
  • Conduct thorough assessments and evaluations of patient care needs related to functional status, activities of daily living, fine motor coordination, and other occupational therapy needs.
  • Develop and implement personalized treatment plans to address patient goals and objectives.
  • Provide interventions aimed at improving and enhancing patient well-being, including home assessments and adaptive equipment recommendations.
  • Evaluate patient progress and make adjustments to treatment plans as needed.
Requirements
  • Current Occupational Therapy licensure in the state of employment.
  • Current CPR certification.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
