Job Title: Caregiving Professional - Resident Support SpecialistAbout the Role:

We are seeking an experienced and compassionate Social Worker to join our team at Regal Heights Rehabilitation & Nursing Center. As a key member of our care team, you will play a vital role in supporting the physical, mental, and psychosocial needs of our residents.

Responsibilities:
  • Maintain and enhance residents' ability to manage daily living skills and address mental/psychosocial needs.
  • Initiate interventions for residents experiencing mental or behavioral health issues.
  • Identify individual social and emotional needs, collaborating with interdisciplinary teams to develop personalized plans.
  • Participate in admission, community re-entry, and transfer planning processes.
  • Work closely with the finance team to ensure accurate documentation and resident education regarding billing and benefits.
  • Coordinate discharge planning, arranging necessary DME and home care services to ensure a smooth transition back into the community.
Qualifications:
  • Nursing Home or Hospital experience; internship accepted.
  • BSW or MSW degree in a related field is required.
  • Bilingual English/Spanish proficiency is preferred but not mandatory.
