REPORTS TO: Program Manager

JOB SUMMARY AND QUALIFICATIONS:

The SSVF program is designed to assist Veterans who are experiencing homelessness or are at risk of losing their housing. The Veteran Support Case Coordinator plays a crucial role in helping veterans overcome their housing challenges, improving their independent living skills, connecting them with essential community resources, and empowering them to achieve housing stability and self-sufficiency.

WHAT YOU SHOULD HAVE FOR THIS ROLE:

This position requires a Bachelor's degree in social work or a related field, with a minimum of five years of relevant experience, or three years of related work experience for Veterans. Candidates must demonstrate proficiency in family needs assessment, advocacy, counseling, and collaboration with local service providers. Strong communication skills, organizational abilities, and problem-solving capabilities are essential. A personal vehicle is required, and the candidate should be prepared to travel frequently.

RESPONSIBILITIES:

  • Conduct outreach to engage veteran families
  • Manage cases, including assessments and the development of case plans
  • Establish connections with service providers
  • Provide referrals and coordinate necessary services
  • Educate participants about available supportive services
  • Complete documentation and input data into HMIS
  • Facilitate the Critical Time Intervention model
  • Collaborate effectively with the SSVF team
