About the Role

We are seeking a skilled Registered Nurse to join our home health team on a part-time basis. As a key member of our care team, you will play a vital role in delivering high-quality patient care and support.

Job Summary

The successful candidate will be responsible for providing admission, case management, and follow-up skilled nursing visits for patients receiving home health services. This includes administering ongoing care and case management, coordinating with healthcare personnel, and implementing patient care plans in conjunction with patients and their families.

Responsibilities
  • Provide admission, case management, and follow-up skilled nursing visits for home health patients
  • Administer ongoing care and case management for each patient, providing necessary follow-up as directed by the Clinical Manager
  • Confer with physicians in developing the initial plan of treatment based on physician's orders and initial patient assessment
  • Coordinate appropriate care, encompassing various healthcare personnel, such as Physical Therapists, Occupational Therapists, Home Health Aides, and external providers
  • Report patient care/condition/progress to patient's physician and Clinical Manager on a continuous basis
  • Implement patient care plans in conjunction with patients and their families to assist them in achieving optimal resolution of needs/problems
  • Coordinate/oversee/supervise the work of Home Health Aides, Certified Home Health Aides, and Personal Care Workers, providing written personal care instructions/care plans that reflect current plans of care
  • Discharge patients after consultation with the physician and Clinical Manager, preparing and completing needed clinical documentation
Requirements

To be considered for this role, applicants must possess:

  • Diploma, Associate or Bachelor Degree in Nursing
  • Minimum of one year nursing experience preferred
  • Strong med-surg, ICU, ER, acute experience
  • Home Health experience a plus
  • Current and unrestricted Registered Nurse licensure
  • Current CPR certification
  • Strong organizational and communication skills
  • A valid driver's license, auto insurance, and reliable transportation are required
