Job Overview

The Puerto Rican Cultural Center is seeking a highly motivated and experienced HIV Prevention Program Coordinator to join our team.

Responsibilities
  • Assist the Program Coordinator with program implementation, including:

    • Providing confidential assessments to determine HIV prevention needs of participants
    • Developing PrEP/PEP resources available in the area for participants
    • Ensuring access, follow-up, and adherence to PrEP/PEP services
  • Collaborate with project staff to advertise and promote the HIV testing program, including geo-social networking

  • Responsible for providing HIV antibody testing on-site and through outreach into the community

  • Provide HIV test results to individuals who have tested for HIV, and HCV

  • Offer Partner Services to individuals who test positive for HIV

  • Perform follow-up risk assessments with individuals who test positive and negative for HIV

  • Maintain communication between inter, and intra-agency services providers for successful coordination of services

  • Document program activities and report outcomes to the CDC as required

Requirements
  • A minimum of two (2) years of experience as a Patient Navigator, Case Management, Education, or Human Services professional

  • Experience in HIV/AIDS services preferred

  • Excellent working knowledge of the New Haven community and social and health service agencies preferred

  • Experience working with LGBTQ, African American, Latino, and low-income populations

  • Specialized training or experience in HIV prevention interventions, PrEP/PEP, and STIs

  • Ability to communicate knowledge about disease (HIV, HCV, and STDs)

  • Knowledge of social services and referrals for patients

  • Must be comfortable working in a clinical setting with providers and medical personnel

Working Conditions

This position requires regular exposure to blood, body fluids, or tissues. The ideal candidate must be able to perform the essential duties satisfactorily.

Compensation and Benefits

The salary for this position is estimated at $40,000 annually, plus benefits, including Health, Vision, Dental, and PTO.
