Job Summary

We are seeking a dedicated Caregiver to provide in-home assistance to our participants with Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs). This is a part-time position, approximately 20 hours per week, with benefits eligibility.

About the Role

The ideal candidate will have experience working with a frail or elderly population and be able to work independently in a minimally supervised setting. They will be responsible for consistently monitoring participant ADL/IADL care plans and following the plan of care as stated. The caregiver will also document care provided on participant flow sheets and report any abnormal findings or alterations in participant status to the team in a timely fashion.

Key Responsibilities
  • Provide in-home assistance with ADLs and IADLs as defined in the participant's care plan
  • Accompany participants to appointments as assigned
  • Carry out simple treatments under the direction of nursing staff, including simple catheter care and skin assessments during personal care
Requirements and Qualifications

To be successful in this role, you will need:

  • Current certification as a H.H.A. /C.N.A. from an approved program (e.g., Red Cross)
  • A minimum of one year experience working with a frail or elderly population
  • Prior experience as a health aide, personal care assistant, or similar role
  • A vehicle to travel from home to home required
  • Excellent customer service and communication skills; bilingual a plus
