Job Summary

We are seeking a highly skilled and compassionate Blended Case Manager to join our team at the Community Guidance Center. As a Blended Case Manager, you will be responsible for providing outreach services to consumers, managing their caseload, and working collaboratively with other staff members to ensure the best possible outcomes.

Key Responsibilities
  • Provide outreach services to consumers, including home visits, community meetings, and office-based services
  • Manage a caseload of 30 consumers, ensuring timely and effective support and services
  • Develop and implement service plans in collaboration with consumers, addressing their strengths and needs
  • Link consumers with community resources and services, promoting independence and self-sufficiency
  • Monitor service delivery, addressing any issues or concerns that may arise
  • Document all interactions and services provided, maintaining accurate and up-to-date records
  • Participate in regular supervision and training sessions to enhance skills and knowledge
Requirements
  • Bachelor's degree in a relevant field, such as social work, psychology, or sociology
  • Or, high school diploma and five years of experience in mental health direct care
  • Act 33 & 34 Clearances and FBI Clearance
  • Act 31 Mandated Reporter Training
  • Excellent communication and interpersonal skills
  • Ability to work in a fast-paced environment, managing multiple priorities and deadlines
  • Valid driver's license and reliable vehicle
