Key Responsibilities

Provide skilled nursing services to patients in their homes, as outlined in their individualized care plans
Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing targeted interventions
Collaborate with interdisciplinary teams to coordinate care and ensure seamless transitions between settings
Educate patients and their families on self-care techniques, medication management, and disease prevention
Maintain accurate and detailed records of patient interactions, progress, and outcomes

Requirements

Current licensure as a Registered Nurse in the state of North Carolina
At least 2 years of experience in home health or hospice care
Strong clinical skills, including infusion therapy and wound care
Excellent communication and interpersonal skills
Ability to work independently and as part of a team
