About the Role

This is a unique opportunity to join our team as a Hospice Nurse - End of Life Care Specialist, providing compassionate care to patients in the West Portland Metro area.

As a member of our team, you will work under physician direction to complete medical requirements while coordinating a whole person plan of care. Your role will involve direct and indirect quality care for patients of all ages, focusing on their individualized needs and following the nursing process.

Key Responsibilities:

  • Providing high-quality, patient-centered care in accordance with the Nurse Practice Act
  • Coordinating patient care activities with other members of the healthcare team
  • Documenting patient information accurately and efficiently
  • Developing and implementing individualized plans of care

Requirements:

  • Oregon Registered Nurse License upon hire
  • 2 years of medical/surgical experience in an acute care setting, or a combination of medical/surgical, long-term care, or home health/hospice experience
  • Experience using electronic medical record software

Salary Information:

The estimated salary range for this position is $50.21 - $70.04 per hour, which may include additional compensation for shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities. This figure is based on industry standards and location.
