Job Title: Geriatric Care Provider

About the Role:

We are seeking a dedicated and compassionate Geriatric Care Provider to join our team at Woodlands Senior Living. As a key member of our healthcare team, you will provide exceptional patient-centered care to our residents, optimizing their function and well-being.

Responsibilities:

* Obtain patient history and perform comprehensive assessments to develop individualized care plans
* Collaborate with multidisciplinary teams to ensure the best possible outcomes for residents
* Provide direct treatment and management of health conditions, including use of prescriptive authority and referral to other healthcare providers and community resources
* Order, perform, and interpret diagnostic studies
* Provide counseling and education to residents and their families concerning preventative health, treatment options, and community resources

Requirements:

* Active, unencumbered license to practice as a Nurse Practitioner in Maine
* Eligibility to practice independently as outlined by the Maine Department of Professional and Financial Regulation Board of Nursing
* Demonstrated experience as an independent Nurse Practitioner preferred
* Current valid license to operate a vehicle in the state of Maine and a driving record that is satisfactory to the company
