Nurse Practitioner/Physician Assistant

Alignment Healthcare is seeking a skilled Nurse Practitioner/Physician Assistant to join our team as a Home Care Specialist. This role will provide medical and social support to our patients in the comfort of their own homes.
Responsibilities- Conduct in-home assessments, medication reviews, and health screenings for Alignment members
- Provide patient education and match care desired with best care given
- Identify diagnoses to be assessed in care management and active medical management
- Partner with Regional Medical Officer and other team members to develop care plans
- Coordinate care with multiple stakeholders, including PCPs, specialists, and ancillary providers
- Lead broader clinical team, including nurses, health coaches, social workers, and care coordinators
- Master's degree from an accredited NP Program or PA program
- Preferred one (1) year of prior clinical or home care experience
- Previous EMR experience preferred
- Active California state Nurse Practitioner or Physician Assistant license
- Active RN License and Furnishing number (Must, upon hire) NP only
- Active Nurse Practitioner Board Certification/Physician Assistant Certification
- NPI Number, DEA, Valid BLS
- Valid California driver license and current automobile insurance
- Yearly Bonus up to 7.5% of annual base salary
- Restricted Stock Unit (RSU) Grant up to 15% of annual salary
- Continuing education reimbursement up to $1,500 per calendar year + 3 CME days
- 18 days of PTO, 8 paid holidays, and 1 Floating Holiday
