Job SummaryThe Medical Navigator will serve as a clinical resource for scheduling coordinators, assisting in identifying appropriate services or providers for referrals. They will also recommend alternative locations of care when necessary and identify discharge needs and services.
Responsibilities- Provide expert clinical guidance to scheduling coordinators to ensure seamless patient care transitions
- Collaborate with multidisciplinary teams to develop personalized care plans
- Identify and address potential barriers to care, such as insurance coverage or social determinants
- Maintain accurate and up-to-date records of patient interactions and outcomes
To be successful in this role, you must possess:
- A current nursing license in good standing
- At least 2 years of experience as a Registered Nurse in a healthcare setting
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- A competitive salary range of $65,000 - $85,000 per year
