Utah Case Manager Professional

We are seeking a dedicated and compassionate Case Manager to join our Assertive Community Treatment (ACT) team at Volunteers of America, Utah.
**About the Role:**
- The Case Manager will provide individualized supportive case management to agency clients with the goal of assisting them in achieving or maintaining stability and independence.
**Responsibilities:**
- Assess service needs and coordinate access to necessary services.
- Apply, acquire, and maintain benefits for clients long-term.
- Consult with community agencies and families to maintain coordination of the treatment process.
- Provide practical help and supports, advocacy, coordination, side-by-side individualized support, problem-solving, direct assistance training, and supervision to help clients obtain the necessities of daily living.
- Provide on-call crisis intervention.
- Participate in daily staff organizational meetings and treatment planning review meetings.
- Complete documentation and meet minimum service time based on ACT requirements.
- Attend work on a regular and predictable basis.
**Requirements:**
- Bachelor's degree in a behavioral science is required.
- Work experience with adults with severe and persistent mental illness and substance use disorders or any combination of education, experience, and training that provides the following knowledge, skills, and abilities:
- Knowledge/experience working with at-risk populations is preferred.
- Knowledge/experience collaborating with community resources.
- Excellent communication skills.
- Must obtain Utah Case Management certification within 60 days of hire.
- Ability to build rapport with clients and engage them in case management.
- Ability to build and foster relationships with partner agencies.
- Ability to maintain a calm demeanor amidst potentially high distress population.
- Basic computer skills including Microsoft Word and Excel spreadsheets.
- Must pass pre-employment drug test.
- Must be at least 21 years of age, possess a current driver's license, have a good driving record, and be insurable on the agency liability policy.
- Must pass Utah DHS - Office of Licensing background screening for those working with vulnerable populations.
**Estimated Salary:** $50,000 - $60,000 per year, depending on experience.
