About the Role

As a Palliative Care Nurse Practitioner at Illumia, you will have the opportunity to make a meaningful difference in the lives of patients and their families. Our team provides personalized care and support to individuals facing advanced illnesses, working closely with physicians and other specialists to deliver comprehensive treatment plans.

Responsibilities
  • Triage patient needs over the phone and respond to crisis calls from patients and families after hours
  • Conduct initial nursing evaluations to determine eligibility for palliative services within 48 hours of referral
  • Identify physical, psychosocial, emotional, and environmental needs of patients and their families, reassessing as needed
  • Collaborate with multidisciplinary teams to provide patient-centered care
  • Document patient progress, goals, interventions, and response to care
Requirements
  • Registered Nurse (RN) license to practice in the state
  • CPR certification
  • Experience in palliative care, oncology, hospice, or geriatric care preferred
  • Fluency in electronic medical record documentation
