Job Title: Substance Abuse Specialist

WellLife Network is seeking a skilled Substance Abuse Specialist to join our team. The ideal candidate will have a Master's Degree in social work, mental health or marriage and family therapy or related field, and two years of experience working with Mental Health and Substance Abuse consumers.

Key Responsibilities:

  • Conduct assessments of dual-diagnosed program participants to identify areas of need.
  • Develop individual service plans for dual-diagnosis consumers and participate in bi-annual reviews.
  • Initiate, conduct, and follow up on periodic drug/alcohol screens.
  • Lead or co-lead psycho-educational/substance abuse educational groups.
  • Facilitate and monitor participation in various self-help groups, including substance abuse support groups, NA, and AA.
  • Coordinate medical, psychiatric, and substance abuse services.
  • Meet with dual-diagnosed participants and management team to review goal progress and identify needs.
  • Participate in inter-disciplinary program planning team meetings.
  • Provide networking and linkages between programs, mental health providers, and alcohol and drug treatment communities.
  • Develop treatment plans with input from other team members and consumers.
  • Coordinate case management services to ensure comprehensive development of individual service plans.
  • Attend and participate in all team meetings.
  • Document progress notes in EHR within 48 hours of client and collateral contacts.
  • Ensure all documentation is completed in a timely fashion and remains in compliance with agency, OMH, and AOT regulations.

Requirements:

  • Master's Degree in social work, mental health or marriage and family therapy or related field.
  • Two years of experience working with Mental Health and Substance Abuse consumers preferred.
  • Experience working with behavioral health population preferred.
  • Computer proficiency, including ability to use email and navigate electronic medical record.
  • Valid New York State driver's license.
