Job Summary:
We are seeking a skilled and compassionate Licensed Practical Nurse to join our team at Blue Mountain Hospital. As a Geriatric Care Specialist, you will be responsible for providing high-quality patient care, with a focus on geriatric patients.

About the Role:
The ideal candidate will have a strong background in geriatric nursing, with experience in caring for patients with complex needs. You will work closely with our interdisciplinary team to develop and implement individualized care plans that prioritize patient comfort, safety, and well-being.

Key Responsibilities:
  • Patient Assessment and Care Planning: Conduct comprehensive assessments of patients' physical and emotional needs, identifying areas for improvement and developing effective care plans.
  • Medication Management: Administer medications as prescribed, monitoring patients' responses and adjusting treatment plans as needed.
  • Wound Care and Pressure Ulcer Prevention: Identify and manage wounds, promoting healing and preventing pressure ulcers through evidence-based practices.
  • Communication and Education: Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination and education on disease management, medication adherence, and self-care techniques.
  • Quality Improvement and Safety: Participate in quality improvement initiatives, contributing to the development and implementation of policies and procedures that promote patient safety and satisfaction.

Requirements:
To succeed in this role, you must possess a current Oregon LPN license, Basic Life Support (BLS) certification, and a minimum of 2 years of experience in geriatric nursing or a related field. A strong understanding of electronic medical records, excellent communication skills, and a commitment to lifelong learning are also essential.
