We are seeking a highly motivated and compassionate Case Manager to join our Interim Housing team at PATH (People Assisting the Homeless). As a Case Manager, you will play a critical role in helping individuals experiencing homelessness achieve housing stability and improve their overall well-being.
Key Responsibilities:
- Conduct intake and individualized needs assessments for clients, collaborating with them to develop a personalized service plan that addresses housing goals, income, and other personal objectives.
- Provide referrals, linkages, information, and support to resources that help clients achieve their service plan goals.
- Complete program-specific assessments, such as the VI-SPDAT, to evaluate strengths and challenges in addressing short-term and long-term goals.
- Evaluate client progress and conduct 90-day reviews to ensure they are on track to meet their housing goals.
- Maintain accurate and up-to-date client files, adhering to documentation standards and confidentiality guidelines.
Requirements:
- Two years of experience in healthcare, case management, homeless services, or a related field.
- Excellent analytical, communication, and problem-solving skills.
- Ability to work effectively with diverse populations and maintain professional boundaries.
