Key Responsibilities

This position is based in Central Phoenix/Glendale area and requires up to 80% travel time.

Job Overview: Arkos Health seeks a Registered Nurse to work with patients and their families, ensuring a smooth transition following discharge from any facility. The RN will be responsible for providing transitional and intensive nurse care management services, working collaboratively with the health plan, leadership, and department coordinators to ensure high-quality patient satisfaction.

Essential Duties and Responsibilities:

  • Work as part of the Arkos Health Interdisciplinary Team, implementing and participating in patient plans of care, updating, and revising as needed.
  • Coordinate comprehensive care, including referrals to other interdisciplinary services (Home Health, Palliative Care, Social Services) to prevent further acute care utilization.
  • Maintain case load based on client needs and/or contractual obligations.
  • Conduct face-to-face visits at least every 2 weeks for assigned patients enrolled in Intensive Case Management, with increased visits based on patient needs and/or contractual obligations.
  • Complete Clinical and Social Assessments (CSA) to develop care plans upon initial start of care visit with patients.
  • Communicate with patient providers and medical specialists regarding patient status and changes in condition.
  • Engage with Arkos Health leadership and contracted providers to resolve patient status changes, complaints, or complications through participation in weekly ICT meetings.
  • Complete medical reconciliation for all transitional care patients within 30 days of initiation of TCM contact.
  • For this field-based position, individuals must be able to travel up to 80% of the time using their personal vehicle.

Requirements and Skills:

  • Current, unencumbered, Registered Nurse License and graduation from an accredited Registered Nursing program required.
  • Current BLS/CPR certification required.
  • Current TB test and maintain a negative test annually required.
  • Hep B vax/titer proof (Company does offer/or may sign a declination) required.
  • Current flu shot (or sign waiver/follow mask policy) required.
  • In-depth knowledge of healthcare services and medical terminology required.
