Primary Care Physician Assistant

Bloom Healthcare, a pioneering and employee-owned primary care and hospice practice, is seeking a skilled Physician Assistant to join our team in Lakewood. As a key member of our care delivery team, you will provide comprehensive primary care at home visits to patients, delivering high-touch, innovative medicine that improves health outcomes.
Responsibilities- Comprehensive Primary Care: Deliver exceptional primary care at home visits to 10 patients per day, managing their health outcomes and providing data-driven, condition-specific specialty visits as needed.
- Communication and Collaboration: Demonstrate exceptional communication and relationship-building skills with care team members, including internal clinicians and external partners, to drive superior patient outcomes.
- Population Health Management: Engage with and appropriately utilize population health data to drive informed decision-making and improve patient care.
To succeed in this role, you must possess:
- A Master's degree from an accredited program in Physician Assistant Studies.
- Licensure in the state of Colorado.
- Bilingualism is preferred but not required.
