Job Summary

We are seeking a skilled and compassionate Community Support Navigator to join our team at Wisconsin Community Services. This role involves providing exceptional support and coordination services to children, young adults, and families in need.

About the Role

The Community Support Navigator will be responsible for facilitating access to various services and support, both formal and informal, that cater to the identified needs of our clients. This includes creating personalized service plans, connecting families with community resources, and promoting self-determination and involvement in community life.

Key Responsibilities:
  • Coordinates and facilitates services with the child and family to meet their developmental, emotional, and physical needs.
  • Provides family-centered, trauma-informed, strength-based, culturally intelligent services through regular collateral contact, direct contact with the family, and face-to-face interactions.
  • Serves as a liaison between families, community agencies, and healthcare providers to ensure seamless coordination of care.
  • Develops and executes comprehensive service plans tailored to the unique needs of each client.
  • Promotes health, safety, and welfare in the home and community environment.
Requirements
  • Bachelor's degree in a health or human services-related field.
  • Minimum one year of experience working with individuals with disabilities.
  • Proficient in Microsoft Office applications and basic computer functions.
  • Able to work effectively in a fast-paced environment and prioritize multiple tasks.
  • Valid driver's license, automobile, and insurance sufficient to meet agency requirements.
