About the Role: We are seeking a highly skilled Advanced Practice Nurse to join our team as a Telehealth Specialist. In this role, you will work in an acute care hospital setting, providing critical care services to patients remotely.

Key Responsibilities:

  • Provide high-quality patient care through remote monitoring and telephonic interventions;
  • Collaborate with interdisciplinary teams to develop and implement individualized patient care plans;
  • Analyze patient data and identify potential complications or concerns;
  • Develop and maintain strong relationships with patients, families, and healthcare providers;
  • Stay up-to-date with the latest clinical guidelines, research, and best practices in telehealth.

Requirements: To be successful in this role, you must have at least two years of experience as an RN or APN, state licensure, and BLS/ACLS certification (at minimum).
