About the Role

This is a dynamic opportunity to make a meaningful impact on the lives of older adults and their families. As a Medical Homecare Professional, you will work collaboratively with our interdisciplinary team to deliver high-end medical services, compassionate care, and industry-leading solutions to vulnerable patient populations.

Duties and Responsibilities

  • Practice as a provider member within the HBMC interdisciplinary team in the direct delivery of home-based medical care to a population within an assigned geography.
  • Collaborate with PCPs, HBMC interdisciplinary teammates, and consultants to develop and carry out patients' goals of care in all phases of the patient journey.
  • Establish and maintain effective relationships with members and their legal representatives, facility staff, PCPs, and other collaborating physicians/providers.
  • Perform initial comprehensive assessments and bi-annual follow-up assessments to develop and update member management care plans.
  • Promote disease management and wellness through education and support for members and facility staff.
  • Participate in case conference reviews, staff meetings, and quality improvement initiatives to ensure the highest standards of care.

Requirements

  • Completion of Nurse Practitioner or Physician Assistant accredited program.
  • Current state RN license (if Nurse Practitioner).
  • Current NP or PA license.
  • Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or the National Commission on Certification of Physician Assistants (NCCPA).
  • Knowledge of long-term care, community resources, and cost-effective alternatives regarding patient care delivery systems.

Compensation and Benefits

The estimated annual salary for this role is $88,000 to $173,200, depending on experience and qualifications. UnitedHealth Group offers a comprehensive benefits package, including incentive programs, equity stock purchase, and 401(k) contributions.
