We are seeking a compassionate and dedicated Licensed Practical/Vocational Nurse to join our team at Compassionate Care Hospice. This is an exciting opportunity to make a meaningful difference in the lives of our patients and their families.

Job Summary

The LVN will be responsible for providing direct patient care under the supervision of a registered nurse, following a comprehensive assessment and plan of care. This includes implementing current nursing practice, providing accurate and timely documentation, and assessing and educating patients and their caregivers on self-care techniques and information pertinent to their diagnosis and plan of care.

Responsibilities
  • Provide direct patient care as defined in the state Nurse Practice Act, including specialized duties related to end-of-life care.
  • Implement the plan of care initiated by the registered nurse, providing accurate and timely documentation consistent with the plan of care.
  • Assess and provide patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care.
  • Participate in coordination of home health services, reporting identified needs to the registered nurse and/or Clinical Supervisor.
  • Use and prepare equipment and supplies effectively, adhering to aseptic technique.
  • Participate in personal and professional growth and development, performing other duties as assigned by the registered nurse.
Requirements
  • Graduate of an accredited practical nurse or vocational nursing program.
  • One year of nursing experience; three years preferred. Community/home health or medical/surgical experience is preferred.
  • Sensitivity to patient needs, with the ability to express patience and compassion in dealing with them.
  • Currently licensed as a Licensed Practical Nurse/Licensed Vocational Nurse in California.
  • Complies with accepted professional standards and practice.
  • Demonstrates good verbal and written communication and organization skills.
  • Possesses and maintains current CPR certification.

The estimated salary range for this position is $30-$35 per hour, negotiable based on experience.
