Job Summary

We are seeking a skilled Social Worker to join our team at Healthcare Nursing Center. As a key member of our interdisciplinary team, you will play a vital role in identifying and addressing the psychosocial needs of our residents.

Responsibilities
  • Conduct thorough social histories and psychosocial assessments to identify residents' needs and develop individualized care plans.
  • Maintain accurate and timely documentation, including social service history, assessments, and progress notes, to ensure compliance with federal and state regulations.
  • Collaborate with the interdisciplinary team to develop and implement care plans that address residents' psychosocial needs.
  • Provide education and support to residents, families, and staff to promote self-advocacy and improve the quality of life for our residents.
  • Assist the Social Service Director in planning, developing, and implementing social service programs.
Requirements
  • Certified, licensed, or registered in the state of practice, required.
  • Bachelor's Degree in Social Work or a related field, required.
  • One year of supervised social work experience in a healthcare setting, required.
  • Minimum of 1 year of nursing experience in a Skilled Nursing Facility, preferred.
