Job Description

We are seeking a highly skilled Renewal Therapist to join our dialysis clinic team in providing psychosocial services to our patients. As a key member of our healthcare team, you will work closely with patients, families, and healthcare professionals to promote positive adjustment, rehabilitation, and improved quality of life.

  • Assess patients' psychosocial status, strengths, and areas of need to identify effective interventions that support rehabilitation and optimal treatment outcomes.
  • Collaborate with the patient and healthcare team to develop personalized care plans that address barriers and meet patient goals.
  • Provide monitoring and interventions to help patients adjust to dialysis and achieve optimal psychosocial status and quality of life.
  • Assist patients with referrals to community resources, home health services, and vocational rehabilitation programs to facilitate optimal treatment outcomes.

Requirements

  • Masters in Social Work degree
  • State-issued license
  • 0-2 years' experience in social work or a related field
