Care Coordinator for Hospice Patients

Cynet Health is seeking a skilled Care Coordinator to provide direct patient care services to hospice patients and their families. As a key member of our interdisciplinary team, you will be responsible for managing patient care services, developing and implementing the Hospice Plan of Care, and providing bereavement support.

**Key Responsibilities:**

  • Develop and implement the Hospice Plan of Care in conjunction with other Interdisciplinary Group members.
  • Provide direct patient care services to hospice patients and supportive care to their families.
  • Manage an interdisciplinary group consisting of Licensed Practical/Vocational Nurses, Certified Nursing Assistants, and support staff to enhance hospice care quality.
  • Assure the highest quality of care is provided and documented in accordance with the Plan of Care.

**Requirements:

  • Current license to practice nursing in the state of employment.
  • BLS (American Heart Association), current automobile insurance, and valid driver's license.
  • Minimum of one year as an RN in acute care hospital, oncology, long-term care, hospice, or home health.
  • Excellent patient assessment and documentation skills.
  • Knowledge of hospice care for terminally ill patients.

**Estimated Salary Range:** $65,000 - $85,000 per year, depending on experience and qualifications. Benefits include comprehensive health insurance, retirement plan, and paid time off.
