Key Responsibilities
  • Conduct thorough assessments of patients' and families' needs, identifying areas for support and intervention.
  • Develop and implement individualized plans of care that address patients' physical, emotional, and social needs.
  • Provide counseling and support to patients and their families, helping them cope with the dying and grieving process.
  • Collaborate with healthcare teams to ensure seamless care coordination and communication.
  • Stay up-to-date with industry developments and best practices in social work and hospice care.
Requirements
  • Master's Degree in Social Work from an accredited institution.
  • Minimum of three years of clinical experience, with one year in a community health setting preferred.
  • LCSW certification and licensure in Maryland and Pennsylvania.
