Home Health Care Nurse

Aveanna Healthcare LLC is seeking a skilled and compassionate Licensed Practical Nurse (LPN/LVN) to join our team of dedicated healthcare professionals. As a Private Duty Nurse, you will provide one-on-one care to medically fragile children and adults in the comfort of their own homes.
Job SummaryThe LPN/LVN will be responsible for delivering high-quality patient care, under the supervision of a Registered Nurse, in accordance with the developed care plan and physician's orders. This is a unique opportunity to make a meaningful impact on the lives of our patients and their families.
Key Responsibilities- Deliver and coordinate quality patient care in compliance with physician orders.
- Continuously observe and assess patient condition and care needs, reporting changes to the supervisor and/or physician as necessary.
- Document all activities, assessments, nursing actions, responses, and coordination of care in a timely manner.
- Participate in and update the nursing care plan to ensure effective patient outcomes.
- Take appropriate nursing action based on assessment and achieve expected outcomes.
- Recognize changes in patient needs and responses requiring intervention, implementing care to prevent risk or reduce risk.
- Graduate of an accredited school of nursing.
- Current, unrestricted state license as a Licensed Nurse in the state of practice.
- Current CPR certification.
- Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
- Six months of recent experience as a Licensed Nurse in a clinical care setting.
- Home health experience.
Aveanna Healthcare LLC offers a competitive salary range of $60,000 - $80,000 per year, depending on experience and location. Additionally, our benefits package includes:
