As a Home Health Physical Therapist, you'll have the opportunity to make a meaningful difference in the lives of patients in their own homes. Our team at Interim HealthCare of New Hampshire is dedicated to providing personalized care and support to individuals struggling with strength and mobility challenges.

About the Role

We are seeking a highly skilled and compassionate Physical Therapist to join our team based out of our Manchester, New Hampshire office. The ideal candidate will hold a valid PT License in the State of New Hampshire and have a strong passion for delivering high-quality care in a home setting.

Key Responsibilities

  • Develop and implement personalized therapy plans to meet the unique needs of each patient
  • Conduct thorough assessments and evaluations to identify areas of improvement
  • Collaborate with our interdisciplinary team to ensure seamless care coordination
  • Provide education and support to patients and their families to promote independence and self-management

Requirements

  • Valid Physical Therapist License in the State of New Hampshire
  • Graduate of an accredited college-level approved program
  • Current CPR certification required
