Job Summary

The Elmcrest Children's Center is seeking a highly skilled Aftercare Clinician to provide clinical services to youth and families transitioning from our residential program. As a key member of our team, you will be responsible for delivering individual, group, and family therapy, as well as coordinating with stakeholders to ensure seamless transitions.

Key Responsibilities
  • Provide Clinical Services: Offer individual, group, and family therapy sessions to youth and families, addressing their unique needs and challenges.
  • Coordinate Transitions: Collaborate with families, county representatives, and other service providers to ensure a smooth transition from our residential program to the community.
  • Develop and Implement Treatment Plans: Create and implement comprehensive treatment plans that address the clinical needs of youth and families.
  • Maintain Accurate Records: Ensure timely and accurate record-keeping, including clinical notes, treatment plans, and progress reports.
  • Communicate Effectively: Develop and maintain positive relationships with families, county representatives, and other stakeholders, ensuring effective communication and collaboration.
Requirements
  • Master's Degree or Higher: A Master's degree in a mental health field (LMSW, LMFT, LMHC, etc.) is required.
  • Experience in Mental Health: A minimum of 2 years of experience working in a mental health environment is preferred.
  • Knowledge of Community Resources: Familiarity with community resources and support systems is essential.
  • Strong Communication Skills: Excellent communication and interpersonal skills are required to work effectively with diverse stakeholders.
