RN Home Health Visiting Nurse 40 Hours a Week Redmond

As a skilled Home Health Visiting Nurse with Kaiser Permanente, you will provide comprehensive nursing care and services to patients in their homes. Working as part of an interdisciplinary team, you will be accountable for assessing patient needs, developing and implementing care plans, and educating patients and their families on health management and disease prevention. Your expertise will be essential in managing pain and symptoms, as well as adjusting to changes in health status. You will also be responsible for planning for the return of patient care to primary healthcare providers as indicated.
Key Responsibilities:
- Collaborate with the interdisciplinary team to plan and implement patient care, ensuring accurate and timely documentation in the electronic medical record.
- Adhere to Kaiser Permanente's infection control, safety, and universal precautions standards.
- Provide skilled nursing interventions, including injections, dressings, catheter insertion and care, colostomy and tracheotomy, central line IV care, and blood draws.
- Develop and implement individualized care plans, including pain and symptom management, and instruct patients and their families on self-care activities and home safety.
- Work independently and as part of the interdisciplinary team to provide comprehensive care and case management.
- Participate in educational efforts to meet your own learning needs and those of the team.
- Adhere to Kaiser Permanente's personnel policies and maintain a high standard of professional work habits.
Requirements:
- Minimum two years of experience as a Registered Nurse in medical-surgical, home health, or hospice settings.
- Associates degree in Nursing from an accredited program.
- Registered Nurse License (Washington) required at hire.
- Basic Life Support required at hire.
- Ability to work independently and as part of the interdisciplinary team.
- Basic laptop skills, including MS Office Suite.
Preferred Qualifications:
- Experience with Horizon for home care system.
- Bachelor's degree in Nursing or a related healthcare field.
