We are seeking a skilled and motivated Licensed Practical Nurse (LPN) to join our healthcare team in Bellevue, WA. As a LPN at Interim HealthCare of Bellevue WA, you will have the opportunity to work in a fast-paced environment and make a meaningful difference in the lives of our patients.

The ideal candidate will possess strong communication and interpersonal skills, with the ability to work well in a dynamic setting. A valid driver's license or dependable access to a vehicle for business use is required. **Job Responsibilities:**

  • Understand and adhere to established policies and procedures
  • Ensure adherence to Universal Precautions for resident and staff safety
  • Prepare residents' medications for short-term absences following state regulations
  • Collaborate with the healthcare team to implement the nursing care plan for each patient
  • Initiate preventive and rehabilitative nursing procedures as appropriate
  • Observe signs and symptoms and report to the physician and RN, including reactions to treatments and changes in the patient's physical or emotional condition
  • Teach and counsel patients and their families/significant others on nursing care needs and related issues for home care
  • Participate in medical record audits as assigned
  • Attend staff meetings, patient care conferences, and in-services as scheduled
  • Read Daily Communication Logs for resident information
  • Document any changes in resident status in Progress Notes and review them regularly
  • Complete resident treatments as ordered and scheduled
  • Assist with personal care and physical support as needed

**Requirements:

  • Licensure: Current unrestricted license to practice as a Licensed Practical Nurse (LPN) in WA state
  • Active BLS (Basic Life Support) certification
  • Preferred minimum of one (1) year of nursing experience
  • Strong communication and interpersonal skills
  • Ability to work well in a fast-paced SNF environment
  • Two (2) positive work references
  • Successfully complete and pass a background check and drug test
