Job Title: Hospice Clinical Liaison

Compassus is seeking a highly skilled and experienced Hospice Clinical Liaison to join our team. As a key member of our clinical team, you will be responsible for promoting the Compassus philosophy and upholding the Code of Ethical Conduct.

Key Responsibilities:
  • Assist referral sources in identifying patients with care needs that match the services provided by hospice.
  • Apply the rules and regulations of state and federal regulatory agencies and other certified agencies.
  • Establish and maintain significant professional relationships with community, hospital, and facility education representatives.
  • Facilitate the admission process to and from hospital and long-term care based referral sources.
  • Assess referred patient's clinical needs and disease status and confer with patient's attending physician and hospice physician regarding assessment to establish eligibility.
Requirements:
  • Associate or Bachelor's degree in Nursing required.
  • Two (2) to three (3) years of related experience required.
  • Hospital clinical experience required.
  • ICU/CCU experience highly preferred.
  • Experience in healthcare marketing preferred.
Skills:
  • Strong written and verbal communications.
  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
  • Ability to write reports, business correspondence, and procedure manuals.
Certifications, Licenses and Registrations:
  • Active and unencumbered Registered Nurse license.
