Clinical Assessment Coordinator

We are seeking a skilled Triage Nurse Specialist to join our team at Colon Rectal Associates of CEN. In this role, you will be responsible for performing a preliminary assessment to determine the acuity level and urgency of medical assistance required, answering patient questions, and serving as a resource and/or mentor to LPNs and Medical Assistants.

• Patient Assessment: Perform patient assessments based on established guidelines, assessing patients over the phone and in the office to determine what type of care they require.
• Triage and Routing: Recommend that the patient seeks emergency treatment, schedules a consultation with a doctor, or treats the condition themselves, and prioritize and route patients to receive appropriate medical care or treatment.
• Medical History Review: Review a patient's medical history prior to receiving treatment, determining the patient's symptoms and severity of illness or injury, and monitoring vital signs.
• Client Care: Provide excellent interpersonal and client care skills, managing the triage telephone system to answer patient questions, and spending time with patients to assist providers with in-office procedures.

• Education and Licensure: Currently licensed Registered Nurse (RN) in the state of NY.
• Experience: 3-5 years of previous experience as a Registered Nurse is preferred.
• Certifications: First aid/CPR certification required.
• Other: Valid driver's license.
