Physical Therapist

We are seeking a skilled and compassionate Home Healthcare Physical Therapist to join our team at Human Touch Home Health Care. The successful candidate will be responsible for providing high-quality physical therapy services to patients in their homes, adhering to our organization's standards and policies.
About the Role:
- Provide physical therapy services to patients according to a written physician's plan of care and as defined in the state Physical Therapy Practice Act.
- Assess and evaluate therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care.
- Direct physical therapy treatment, instruct patients and families/caregivers in the use and care of therapeutic appliances, and determine priority needs for physical therapy.
- Report to physician patient's reaction to treatment or changes in condition, and initiate physical therapy program as needed.
Requirements:
- Possess a degree from a baccalaureate or master's program in physical therapy approved by an accredited organization.
- Licensed to practice as a physical therapist within the state, if applicable.
- Two years of appropriate experience as a physical therapist, with community/home health experience preferred.
- Demonstrate good verbal and written communication and organization skills, and possess current CPR certification.
- Licensed driver with automobile that is in good working order and insured in accordance with the organization's requirements.
