Job Description

The Nurse Practitioner (APRN) at Abode Care Partners provides evidence-based medical care to seniors and patients with complex needs. With a focus on interdisciplinary collaboration, this role involves performing medical evaluations, ordering diagnostic tests, and working with other healthcare professionals to ensure comprehensive patient care.

Key Responsibilities
  • Perform medical evaluations and develop treatment plans for patients
  • Order laboratory and diagnostic tests as necessary
  • Collaborate with caregivers and other healthcare professionals to provide interdisciplinary care
  • Ensure patient medical records are up-to-date
Required Skills and Qualifications
  • Active state Nurse Practitioners license
  • Current Nurse Practitioner National Board Certification
  • Active state DEA licensure
  • Master's degree in Nursing (MSN)
  • Five or more years of nursing experience; at least two years of nurse practitioner experience preferred
