About the Job

We are seeking a highly skilled Care Coordination Specialist to join our team at CAI in providing exceptional geriatric care services. As a key member of our care coordination team, you will play a vital role in ensuring seamless transitions between healthcare settings and delivering high-quality care to our clients.

About the Role

The Care Coordination Specialist will be responsible for assessing and addressing the complex health needs of our clients, developing and implementing individualized care plans, and facilitating communication among healthcare providers and stakeholders. You will work closely with our interdisciplinary team to ensure that each client receives comprehensive and coordinated care, resulting in optimal health outcomes.

Key Responsibilities
  • Assess and Develop Care Plans: Conduct thorough assessments of client needs, develop personalized care plans, and coordinate services to meet those needs.
  • Facilitate Transitions: Ensure smooth transitions between healthcare settings, including hospitalizations, discharges, and transfers.
  • Communicate Effectively: Foster strong relationships with healthcare providers, family members, and caregivers to ensure effective communication and coordination of care.
  • Maintain Accurate Records: Document all interactions, assessments, and care plan updates in our electronic health record system.
Requirements
  • Education and Experience: A current Registered Nurse license in the state of Delaware and 5+ years of experience in case management and discharge planning, with a focus on geriatric care.
  • Skills and Qualifications: Excellent assessment and communication skills, ability to work independently and as part of a team, proficiency in electronic documentation systems, and a commitment to delivering high-quality care.
Compensation

The estimated salary for this role is $65,000 - $85,000 per year, depending on experience and qualifications. We offer a competitive benefits package, including medical, dental, and vision coverage, retirement savings plan, and paid time off.
