Home Health Therapist Opportunity in Vermont

BAYADA Home Health Care is a leading provider of home health care services, seeking an experienced Occupational Therapist to join our team in Williston, VT. This role offers a unique opportunity to make a meaningful impact on the lives of adult and geriatric patients in their own homes.
Job SummaryWe are looking for a highly skilled and compassionate Occupational Therapist to perform home visits, providing rehabilitative care and skilled nursing services to clients in Washington County. As a member of our multidisciplinary healthcare team, you will work collaboratively with nurses, social workers, and other professionals to develop individualized care plans that address the unique needs of each patient.
Responsibilities- Conduct thorough evaluations of patients' functional status, including muscle function, endurance, visual coordination, written and verbal communication skills, and ADL and IADL performance.
- Develop and implement individualized intervention plans to achieve favorable outcomes, taking into account cognitive, psychological, and motor aspects of performance.
- Evaluate the home environment for hazards or barriers to independent living and identify equipment needs and environmental adaptations.
- Continuously assess and revise the occupational therapy care plan, participating with nursing in the multidisciplinary care plan as appropriate.
- Educate and instruct patients, family members, or representatives on rehabilitative care and activities necessary to promote health, safety, and independent living.
- Accurately document observations, interventions, and evaluations using state-of-the-art technology.
- A current Vermont Occupational Therapist license.
- Prior home care experience preferred but not required.
- A minimum of one year of recent work experience as an Occupational Therapist.
- Graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA.
- Ability to work independently and manage time effectively.
- Strong interpersonal skills, organizational skills, and problem-solving skills.
- Solid computer skills, prior experience with electronic medical records (EMR) preferred.
- Competitive salary: $75,000 - $90,000 per year, depending on experience.
