We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Adoration Home Health & Hospice. This part-time position offers a consistent weekday schedule, allowing you to make a meaningful difference in patient care.

About the Role

This LPN role involves providing one-on-one in-home care for a patient requiring G-Tube support. You will be responsible for performing selected nursing procedures within the scope of LPN practice, providing patient and family teaching according to plan outlined by the RN, and monitoring patient's vital signs and making specific measurements and observations.

Responsibilities
  • Perform selected nursing procedures within the scope of LPN practice
  • Provide patient and family teaching according to plan outlined by the RN
  • Monitor patient's vital signs and make specific measurements and observations
  • Monitor and record intake and output, reporting significant changes immediately
  • Observe, assess and record patient's physical and mental condition and report any changes
  • Administer medications as instructed and as ordered by the physician and instructed by the RN, and document the patient's file accordingly
Qualifications
  • Licensed Practical Nurse/LPN license in good standing
  • Previous private duty, home health, hospice, hospital or skilled nursing experience preferred
  • Active CPR certification
  • Familiarity with electronic health records
  • Strong problem-solving and critical thinking skills
  • Reliable transportation, valid driver's license, and current automobile insurance
