About the Role

This LPN position is part of our skilled nursing team at Huntersville Oaks, where we strive to create a warm and welcoming environment for our patients and families. The successful candidate will be responsible for performing various aspects of patient care, including assessments, documentation, and implementation of care plans under the supervision of a registered nurse or physician.

Responsibilities
  • Conduct thorough assessments of patients/residents to identify their physical, emotional, and social needs.
  • Develop and implement individualized care plans that address the unique needs of each patient/resident.
  • Collaborate with the healthcare team to ensure seamless communication and coordination of care.
  • Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by the physician.
  • Maintain accurate and detailed records of patient interactions, observations, and treatment outcomes.
Requirements
  • Graduate from an accredited School of Practical Nursing.
  • Holding a current LPN license or temporary license as a Licensed Practical Nurse Petitioner in the state where you work and reside.
  • BLS certification per policy guidelines.
  • Demonstrated ability to work effectively in a fast-paced environment and prioritize tasks accordingly.
