Family Support Navigator

Bay Cove Human Services is seeking a Family Support Navigator to join our team.

Job Summary:

The Family Support program aims to help families stay together by developing their capacity to meet their needs, offering additional supports, and enhancing community capacity to value and support people with disabilities and their families.

The Family Support Navigator will ensure that all families are treated with dignity and respect, and given the opportunity to reach their goals through case management and coordination.

Essential Functions:

  • Respond to inquiries to the Family Support Center as a first responder
  • Provide case management to families through the Family Support Center, including assessments, short-term and extended navigation services, financial allocation support, and resource assistance
  • Assist families in developing and implementing Family Support Plans
  • Provide training to family members using intervention strategies in the home/community
  • Facilitate support groups for families through the Family Support Center
  • Recruit and train staff for home-based services
  • Oversee home-based services through supervision and case management of DDS cases
  • Serve as a liaison between family, in-home worker, and DDS
  • Provide assistance in locating community resources and referrals as appropriate
  • Act as a liaison with other service providers, such as special education programs and healthcare providers
  • Maintain documentation according to program and funding requirements
  • Attend internal and external meetings as needed
  • Perform other duties as assigned by supervisor

Qualifications:

  • Bachelor's degree in social work or related field
  • Five years of experience in human services or related field preferred
  • Valid driver's license required
  • Use of personal vehicle needed for transportation to clients' homes, appointments, meetings, and to perform varied work duties as needed
  • Use of personal cellular telephone for work communication
  • Bilingual in English and Spanish preferred but not required
