Job Overview

We are seeking a highly skilled Comprehensive Care Manager to join our team at The Good Seed CDC. This is a full-time position that requires a strong background in community health and social services.

About the Role

The Comprehensive Care Manager will be responsible for coordinating and implementing organization-wide Enhanced Care Management. This involves overseeing and providing the Enhanced Care Management (ECM) services, as well as identifying and achieving Care Plan goals and objectives with members that meet their self-identified strengths and healthcare needs.

Key Responsibilities:
  • Engage patients and offer and/or facilitate care management services where the patient lives, seeks care, or finds most easily accessible.
  • Conduct comprehensive risk assessments and develop patient-centered Care Plans that include goals based on the patients' physical and psychosocial health needs and consider their personal preferences.
  • Oversee effective implementation of Care Plans, ensuring initial plans are drafted within 30 days from patient enrollment and updated as necessary, but no less than one per quarter thereafter.
  • Educate patients on self-management skills and/or recruit support from a caregiver/family member to support the accomplishment of the Care Plan.
  • Support health behavior change utilizing motivational interviewing and trauma-informed care practices.
  • Monitor treatment adherence.
  • Regularly initiate or participate in case conferences with clinical providers.
  • Connect patients to social services, including housing, transportation, etc., as needed to achieve patients' goals and well-managed care.
  • Coordinate with hospital staff on discharge plans and other transitional care as feasible.
  • Maintain regular contact schedules with enrolled patients, including at least one in-person encounter per month.
Requirements:
  • Bachelor's degree in Social Services preferred.
  • At least two years of experience in community health or social service settings required.
  • Two to three years of case management/care coordination experience preferred.
  • Bilingualism in English and Spanish or Armenian fluency is required.
  • Proficiency in Microsoft Office Suite products.
  • A valid driver's license and willingness to drive to communities where ECM members live.
  • Ability to work independently and collaboratively as part of an interdisciplinary team.
Salary and Benefits

The estimated salary for this position is $65,000-$75,000 per year, depending on experience and qualifications. Benefits include paid holidays/PTO schedule, 8-hour shifts, Monday to Friday work hours, and opportunities for professional growth and development.
