Job Summary

We are seeking a highly skilled and compassionate Care Coordinator to join our team at Mental Health Resources. As a Care Coordinator, you will play a vital role in managing the needs of our clients and providing guidance, empathy, education, and support.

Key Responsibilities
  • Engage with clients routinely, building rapport and trust to ensure effective communication and collaboration.
  • Educate family members about the client's physical and behavioral needs, working closely with them to understand, accept, and follow medical recommendations.
  • Administer and complete the initial Comprehensive Needs Assessment (CNA) to identify the client's needs and develop a personalized care plan.
  • Document coordinated activities through BHSD Star and EHR, ensuring accurate and timely recording of client interactions and progress.
  • Promote the principles of recovery and resiliency, empowering clients to take control of their mental health and well-being.
  • Collaborate with health professionals to ensure clients receive the proper care, including making appointments and attending appointments.
  • Organize and create care plans to help clients achieve their physical and behavioral goals, utilizing community resources to enhance their quality of life.
  • Identify risk factors and maintain HIPAA and client confidentiality at all times.
Requirements
  • Bachelor's degree in any human service field.
  • Strong communication and interpersonal skills, with the ability to build rapport with clients and their families.
  • Ability to work independently and as part of a team, with a focus on collaboration and coordination.
  • Proficiency in using electronic health records (EHR) and other software applications.
  • Strong problem-solving and critical thinking skills, with the ability to analyze complex situations and develop effective solutions.
