Job Description:Job Summary: We are seeking a skilled Primary Care Nurse Practitioner to provide high-quality patient care in a home health setting. The ideal candidate will have experience in geriatric care and be able to work independently with minimal supervision.
Responsibilities:
- Conduct in-home health and wellness assessments for health insurance plans
- Perform H&P evaluations, review medications, and conduct physical exams
- Collect urine samples and fecal kits for patient mail-in testing
- Document patient information in a proprietary EMR system
- Meet productivity requirements of 5-8 assessments per day
Requirements:
- CAQH and NPI # registration
- Unrestricted NP license in the state of practice
- Malpractice insurance coverage
