About the Role

We are seeking an experienced Licensed Practical Nurse to join our team at The Oaks. This is a rewarding opportunity to make a meaningful difference in the lives of our residents.

Job Description
  • Assess patients in collaboration with Registered Nurses and administer medications as prescribed.
  • Perform routine lab tests and therapeutic services, ensuring the highest level of care for our residents.
  • Educate patients and their families on disease treatment plans, including self-care post-discharge and available resources.

Promoting Resident Safety and Well-being

  • Identify safety hazards and emergency situations, taking corrective action to ensure resident safety.
  • Develop and implement effective care plans, prioritizing resident-centered care.
